North York Toronto Health Partners recognized for outstanding leadership in community health system awards


NYTHP Lead Community Health Ambassador Teresina Stanichevsky accepted the Health System Champion Award at FHC's AGM and Community Celebration on September 17th, 2024.


North York Toronto Health Partners (NYTHP) has been awarded the 2024 Health System Champion Award by Flemingdon Health Centre (FHC). Lead Community Health Ambassador, Teresina Stanichevsky, received the award on behalf of NYTHP at FHC’s AGM and Community Celebration hosted at The Japanese Cultural Centre on September 17th, 2024.


FHC recognized NYTHP for its Community Health and Information Fairs (CHIFs), which offer low-barrier access to preventative health care, community and social services to the people of North York. NYTHP works in partnership with local primary care providers, community health centres, health care, community and social services to deliver care directly to the community.


“We are very happy to receive this award as we continue to expand accessible and equitable care programs through our community-based partners, including Flemingdon Health Centre, who have been an important partner in helping us deliver care in the East end of North York,” said Teresina. “These partnerships are what allow us to offer care and education to members of our community that need them the most.”



FHC’s Health System Champion Award honours individuals and/or organizations who have shown outstanding leadership and dedication to improving community health as a whole rather than the advancement of their own organization individually. Past recipients include Michael Garron Hospital, The Neighbourhood Organization and South Riverdale Community Health Centre.


“We are excited to present this award to celebrate health care system leaders who are going above and beyond to help those experiencing the greatest needs,” said Jen Quinlan, CEO at Flemingdon Health Centre. “The CHIFs are an excellent example of our collective impact as an Ontario Health Team and showcase what is possible through collaborations across the continuum of care.”


The CHIFs are co-designed with community partners to improve health and wellness in high-needs neighbourhoods in North York. They were developed to support a wide range of health and social needs and provide tailored offerings ranging from blood pressure and blood glucose checks to cancer screening programs, including cervical cancer screening (Pap tests) on-site with a female primary care provider and the opportunity to be referred to breast cancer screening and colorectal cancer screening.


Flemingdon Health Centre is a community-based, non-profit organization that provides free health care services and community programs to residents.
